Goals

The Systems Transformation Grant and Money Follows the Person Demonstration Project are being implemented through four workgroups – each tasked with implementing a specific goal:

• Improving Access to Long Term Support Services through the Development of a No Wrong Door One-Stop System

 

• Increased Choice and Control with the Development and Enhancement of a Person-Centered and Self-Directed System

 

• Improving Service Delivery through Modernized Information Technology Systems

 

• Providing options for over 1,000 individuals to transition from a long-term care facility to the community under the Money Follows the Person Demonstration Project
